Overview

Intestinal osmotic mechanisms refer to the physiological processes governing the movement of water across the intestinal epithelium driven by osmotic pressure gradients (StatPearls, 2023). This mechanism is primarily utilized by osmotic laxatives, which consist of poorly absorbed salts or sugars that remain within the intestinal lumen (NIH, 2023). By increasing the solute concentration in the gut, these agents prevent water reabsorption and draw additional fluid into the bowel through osmosis (Mayo Clinic, 2023). The resulting increase in fecal volume and softening of stool consistency stimulates intestinal motility and facilitates defecation (StatPearls, 2023). Beyond simple constipation, these mechanisms are targeted for bowel cleansing prior to medical procedures and for managing hepatic encephalopathy by trapping ammonia in the gut (NIH, 2023). While generally safe, manipulation of these mechanisms requires careful monitoring of fluid and electrolyte balance, particularly in patients with renal or cardiac comorbidities (NIH, 2023).

Mechanism of action

Osmotic agents are poorly absorbed solutes that remain in the intestinal lumen, creating an osmotic gradient that draws water into the bowel. This increases intraluminal pressure, softens stool, and stimulates peristalsis (StatPearls, 2023).
