Overview

Intestinal water refers to the aqueous medium within the lumen of the gastrointestinal tract, primarily the small and large intestines. It serves as the essential solvent for the digestion and absorption of nutrients and the transport of waste products (NCBI, 2022). While not a molecular target like a receptor or enzyme, intestinal water volume is a key physiological parameter modulated by various pharmacological agents to treat motility disorders. Osmotic laxatives, such as polyethylene glycol, increase the volume of intestinal water by creating an osmotic gradient that prevents water reabsorption (PubChem). Conversely, secretagogues like linaclotide increase luminal water by activating the cystic fibrosis transmembrane conductance regulator (CFTR) to secrete chloride and bicarbonate (PubMed, 2014). Proper regulation of intestinal water is vital for maintaining systemic fluid balance and preventing conditions such as constipation or life-threatening diarrhea.

Mechanism of action

Pharmacological agents manipulate intestinal water by creating osmotic gradients to retain fluid or by stimulating ion channels (e.g., CFTR, ClC-2) to increase fluid secretion into the lumen (StatPearls, 2023; PubMed, 2014).
