Overview

Intra-bone cord blood hematopoietic stem cell transplantation (IB-CB-HSCT) is a clinical procedure involving the direct injection of umbilical cord blood (UCB) into the bone marrow space, usually the posterior superior iliac crest (Frassoni et al., 2008, Lancet Oncology). This technique is designed to improve the efficiency of hematopoietic stem cell (HSC) engraftment by bypassing the pulmonary and hepatic sequestration that occurs during traditional intravenous administration (Rocha et al., 2013, Biology of Blood and Marrow Transplantation). By delivering the limited number of HSCs found in a single cord blood unit directly into the marrow niche, the procedure aims to accelerate the recovery of neutrophils and platelets (Brunstein et al., 2009, Blood). IB-CB-HSCT is primarily utilized in the treatment of high-risk hematologic malignancies and bone marrow failure syndromes where a suitable HLA-matched adult donor is unavailable (Saccardi et al., 2016, Bone Marrow Transplantation). Despite its clinical utility, it is a delivery methodology and not a molecular target such as a receptor or enzyme.
