Overview

The intracellular calcium-sensing machinery is a complex network of proteins that transduce changes in cytosolic calcium levels into specific cellular responses. This machinery includes high-affinity sensors like Calmodulin, Synaptotagmin-7, and Doc2, which are particularly sensitive to 'bulk' or global calcium rises that occur throughout the cytoplasm (NIH, 2023; BioRxiv, 2018). These sensors regulate a variety of downstream effectors, including kinases (e.g., CaMKII), phosphatases (e.g., calcineurin), and the exocytotic apparatus, thereby controlling processes such as neurotransmitter release, muscle contraction, and gene expression (Frontiers, 2010; PNAS, 2011). In the brain, the bulk-sensitive machinery is essential for asynchronous release and synaptic facilitation, and its dysfunction is linked to neurodegenerative disorders and epilepsy. Pharmacological intervention targeting this machinery includes calcineurin inhibitors like cyclosporine and synaptic vesicle protein modulators like levetiracetam (StatPearls, 2023). However, the fundamental role of calcium in almost all cell types makes this machinery a challenging therapeutic target due to the high risk of systemic side effects and toxicity.
