Overview

Intracellular water (ICW), also termed intracellular fluid (ICF), represents the largest fluid compartment in the human body, accounting for roughly two-thirds of total body water (StatPearls, 2023). It serves as the essential aqueous solvent for all internal biochemical reactions, metabolic processes, and the maintenance of cellular structural integrity via turgor pressure (NIH, 2021). While ICW is not a traditional molecular drug target—such as a specific protein receptor or enzyme—its volume and tonicity are critical clinical parameters regulated by the balance of osmotic forces and the activity of aquaporin water channels (PubMed, 2022). Pathological alterations in ICW volume, specifically cellular swelling or shrinkage, are hallmark features of metabolic and neurological disorders such as cerebral edema, hyponatremia, and diabetic ketoacidosis (StatPearls, 2023). Therapeutic interventions typically manage ICW by modulating the extracellular environment to induce osmotic shifts (e.g., using mannitol) or by targeting the hormonal pathways, such as the vasopressin system, that govern systemic water homeostasis (NIH, 2022).
