Overview

Intrahepatic lipid accumulation is the excessive storage of fats, mainly triglycerides, within liver cells. Normally, the liver plays a key role in lipid metabolism by orchestrating fatty acid uptake, synthesis (de novo lipogenesis), oxidation, and export as very low-density lipoproteins (VLDL). When the rate of lipid acquisition exceeds disposal, such as in metabolic syndrome or insulin resistance, fat builds up, driving conditions like non-alcoholic fatty liver disease (NAFLD) and non-alcoholic steatohepatitis (NASH). Accumulation may impair liver function, promote inflammation, fibrosis, and eventually progress to cirrhosis or cancer. Drugs aim to influence upstream regulators, metabolic enzymes, or transcription factors that ultimately reduce hepatic fat burden, rather than targeting 'intrahepatic lipid accumulation' as a discrete molecular entity. Intrahepatic lipid accumulation is not a molecule or receptor target, but a pathophysiological process involved in several liver diseases, particularly NAFLD and NASH. It results from imbalance between lipid uptake, synthesis, oxidation, and export in liver cells. Therapies target metabolic pathways upstream rather than the physical presence of liver fat itself.

Mechanism of action

Drugs do not directly interact with intrahepatic lipid accumulation itself, but rather influence upstream regulators, metabolic enzymes, or transcription factors. Mechanisms often include: reduction of de novo lipogenesis, enhancement of fatty acid oxidation, inhibition of fatty acid uptake, improvement of lipid export, and improvement of insulin sensitivity.
