Overview

The intrinsic tenase complex is a critical multi-component enzyme assembly in the blood coagulation cascade, primarily responsible for the efficient conversion of Factor X to its active form, Factor Xa (StatPearls, Coagulation Cascade). This complex is composed of the serine protease Factor IXa, the non-enzymatic cofactor Factor VIIIa, calcium ions, and a procoagulant phospholipid surface, typically provided by activated platelets (UniProt P00740, P00451). Within the intrinsic pathway, the assembly of this complex increases the catalytic efficiency of Factor X activation by several orders of magnitude compared to Factor IXa alone. Deficiencies in the components of this complex lead to severe bleeding disorders, most notably Hemophilia A (Factor VIII deficiency) and Hemophilia B (Factor IX deficiency). Modern therapeutic strategies, such as the bispecific antibody emicizumab, target this pathway by mimicking the spatial orientation provided by Factor VIIIa to bridge Factor IXa and Factor X, thus restoring hemostatic function in patients (PubMed PMID: 27558864). Monitoring the activity of this complex is essential for managing bleeding risks and evaluating the efficacy of mimetic therapies.
