Ion transport systems are a broad class of integral membrane proteins, including ion channels, pumps (ATPases), and transporters, that facilitate the movement of ions across biological membranes to maintain electrochemical gradients (StatPearls, 2023). These systems are critical for fundamental physiological functions such as signal transduction in the nervous system, muscle contraction, and the regulation of cellular volume and pH (NCBI, 2022). Dysregulation of these proteins is implicated in a wide variety of disorders collectively known as channelopathies, as well as cardiovascular, metabolic, and neurodegenerative diseases (PubMed, 2021). Pharmacological agents targeting these systems include a diverse range of drugs such as diuretics, anti-arrhythmics, and proton pump inhibitors, which act by modulating ion flux to restore physiological balance (PubChem, 2023). However, because many of these proteins are ubiquitously expressed, a major challenge in drug development is achieving tissue and isoform specificity to avoid systemic safety concerns like electrolyte imbalances or cardiac arrhythmias (NIH, 2023).
Drugs targeting ion transport systems typically act by physically blocking the pore of ion channels, competitively or non-competitively inhibiting transporter binding sites, or modulating the gating properties and kinetics of the protein to increase or decrease ion flux across the cell membrane.
