Overview

Ipratropium bromide is a quaternary ammonium derivative of atropine used primarily as an inhaled anticholinergic (muscarinic antagonist) for the treatment of chronic obstructive pulmonary disease (COPD), asthma exacerbations, and symptomatic relief of rhinorrhea. It acts by blocking muscarinic acetylcholine receptors (targets: M1-M5) in bronchial smooth muscle, causing bronchodilation and reducing mucus secretion. It has limited systemic absorption and produces fewer cardiovascular side effects than atropine, making it suitable for inhalational use. Ipratropium bromide is commonly used in combination with beta-2 adrenergic agonists (such as albuterol/salbutamol or fenoterol) for synergistic bronchodilatory effect[1][2][3][4][7][8][9].
