Overview

Ipsilateral cortical motor neurons are neurons residing in the motor cortex (primarily M1, SMA, and premotor areas) that are involved in the planning and/or execution of movements on the same side of the body as their cortical origin. Unlike the classic cross-body (contralateral) control, ipsilateral motor cortex activation can occur during complex, bilateral, or unimanual tasks and is modulated by task complexity and accuracy demands. The connectivity and activation patterns of these neurons are distinct and often orthogonal in network space from contralateral activity, allowing for flexible bimanual coordination, movement planning, and adaptive responses after injury. Although their activity may not directly induce same-side motor output in healthy adults, changes in their network involvement are important for recovery and compensation after CNS injuries such as stroke. They are not a molecular drug target, but represent a functional/anatomical domain relevant for neurophysiology and neurorehabilitation research.
