The ischemic penumbra is a region of brain tissue surrounding an infarct core that has reduced blood flow but remains metabolically active and potentially salvageable (StatPearls, NBK537105). It is characterized by a state of electrical silence where neurons are alive but non-functional due to moderate ischemia, typically defined by cerebral blood flow levels between the thresholds of functional impairment and structural integrity (PubMed, 15105596). This zone is the primary target for acute stroke interventions, such as thrombolysis with Alteplase or mechanical thrombectomy, which aim to restore perfusion before the tissue progresses to irreversible infarction (NIH, Stroke Treatment). The survival of the penumbra depends heavily on the duration of ischemia and the adequacy of collateral blood flow from adjacent vascular territories.
