Overview

Ischemic tissue is a pathological state characterized by an inadequate supply of oxygenated blood to a specific organ or region, resulting in a mismatch between metabolic demand and nutrient supply (StatPearls, 2023). This condition initiates a rapid decline in cellular ATP levels, forcing a shift to anaerobic metabolism and causing intracellular acidosis and ion pump failure (NIH, 2024). Prolonged ischemia leads to irreversible cell death, typically through necrosis or apoptosis, and triggers a robust inflammatory response (PubMed, PMID: 28636350). In clinical practice, ischemic tissue is the focal point for treatments such as thrombolysis or angioplasty, which aim to restore perfusion. However, the reintroduction of oxygen can paradoxically cause further damage, known as reperfusion injury, through the generation of reactive oxygen species (Nature Reviews Cardiology, 2019). While not a molecular target itself, it represents the physiological environment where various molecular targets, such as HIF-1α or adenosine receptors, are modulated for therapeutic benefit.
