Overview

The joint cartilage and synovial surface extracellular matrix (ECM) is a complex, highly organized network of proteins and glycans that provides structural integrity and low-friction movement to diarthrodial joints (Sophia Fox et al., 2009, Sports Health). Its primary components include type II collagen, which provides tensile strength, and aggrecan, a large proteoglycan that resists compression through its high osmotic pressure (Heinegård, 2011, Int J Exp Pathol). The synovial surface is further coated with lubricating molecules like lubricin (PRG4) and hyaluronic acid, which minimize wear during joint articulation (Jay & Waller, 2014, Matrix Biol). In diseases such as osteoarthritis, the balance between ECM synthesis and degradation is disrupted, leading to the loss of these structural components and subsequent joint failure (Little & Fosang, 2010, Nat Rev Rheumatol). Therapeutic strategies targeting the ECM include the use of viscosupplements to restore lubrication, growth factors to stimulate matrix production, and inhibitors of matrix-degrading enzymes like MMPs and ADAMTS (Lohmander et al., 2014, Arthritis Rheumatol).
