Keratin 20 is a type I cytokeratin and intermediate filament protein encoded by the KRT20 gene. It is primarily expressed in mature enterocytes and goblet cells of the gastric and intestinal mucosa, as well as in urothelium, bladder, and Merkel cells. It plays a crucial structural role in maintaining the integrity of epithelial cells and is involved in organizing intermediate filaments in intestinal epithelia. Keratin 20 shows restricted tissue distribution and serves as a reliable marker for the differentiation state of epithelial cells, especially in the gastrointestinal tract. Its expression patterns are widely used as diagnostic and prognostic markers in several cancers, including colorectal and urothelial carcinomas. However, it is not considered a direct therapeutic target such as a receptor, enzyme, or transporter.
