Keratolytic activity refers to the pharmacological process of softening and shedding the stratum corneum, the outermost layer of the epidermis composed of keratinized cells. This activity is vital for treating hyperkeratotic skin conditions, such as psoriasis, acne, and warts, where the natural process of desquamation is impaired or where excess skin buildup occurs (StatPearls, NBK499879). Keratolytic agents like salicylic acid and urea achieve this by reducing the adhesion between corneocytes, typically by disrupting intercellular lipids or dissolving the proteinaceous bridges (desmosomes) that hold the cells together (NIH, PubChem). While clinically significant, keratolytic activity is a descriptive therapeutic effect or mechanism of action rather than a specific biological target molecule like a receptor or enzyme. It is commonly utilized in dermatology to facilitate the penetration of other topical medications and to improve the appearance and texture of the skin. However, the use of substances with high keratolytic activity requires careful management to prevent excessive skin irritation or systemic absorption of the active compounds (StatPearls, NBK544260).

Mechanism of action

Keratolytic agents work by dissolving the intercellular cement (desmosomes) that binds corneocytes together, increasing the water-binding capacity of the stratum corneum, or directly breaking down keratin proteins, which facilitates the shedding of dead skin cells.
