The kidney glomerulus filtration barrier is a highly specialized, multi-layered structure within the renal glomerulus that serves as the primary site for blood filtration in the kidney. Its main function is to allow water and small- to midsized solutes to pass from the bloodstream into Bowman’s capsule while restricting larger molecules, particularly proteins such as albumin, thereby maintaining plasma protein homeostasis. It is composed of fenestrated endothelial cells, the glomerular basement membrane (GBM), and podocytes with slit diaphragms. Damage or dysfunction at any layer can lead to increased permeability for macromolecules—manifesting clinically as proteinuria—and contributes significantly to progression in various kidney diseases.
