Overview

The lateral prefrontal cortex (LPFC), which includes the dorsolateral (DLPFC) and ventrolateral (VLPFC) subregions, is a primary anatomical hub for high-order executive processing and cognitive control (NIH/NIMH). It plays a vital role in managing working memory, shifting attention, and integrating complex information to guide goal-directed behavior (PubMed: 28823432). In clinical psychiatry, 'activity' in this area is frequently used as a functional biomarker; for example, 'hypofrontality' or reduced LPFC activity is a hallmark of the cognitive deficits seen in schizophrenia and the emotional dysregulation found in major depressive disorder (StatPearls: Schizophrenia). While not a single molecular entity, the LPFC is a 'systems-level target' for interventions such as repetitive Transcranial Magnetic Stimulation (rTMS), which is FDA-approved for treating depression by stimulating the DLPFC (Mayo Clinic). Pharmacological treatments, including stimulants for ADHD and antidepressants, indirectly modulate LPFC activity by altering the levels of catecholamines and other neurotransmitters that influence local pyramidal cell firing and circuit connectivity (PubMed: 17311305). Consequently, the LPFC remains a focal point for developing both focal neuromodulation therapies and system-wide psychotropic drugs.
