Overview

Leucine-rich repeat-containing protein 23 (LRRC23) is a protein coded by the *LRRC23* gene in humans, primarily expressed in the testis and localizing to the sperm flagellum[1][5]. LRRC23 is a member of the leucine-rich repeat (LRR) family, which function predominantly as scaffolds for protein-protein interactions[1][7]. Biologically, LRRC23 is essential for male fertility by ensuring proper assembly of the RS3 (radial spoke 3) head and the bridge structure between RS2 and RS3 in the flagella of sperm cells[2][4][5]. Mutations or truncations in LRRC23 disrupt the assembly of the RS3 head, leading to defective flagellar motility and resulting in male infertility, specifically classified as Spermatogenic Failure 92[2][4][5]. While LRRC23 is not a recognized therapeutic target and has no known drug interactions, it is critical for understanding the mechanisms underlying sperm motility disorders and infertility. The protein shows a typical LRR architecture of alternating beta sheets and coils that form a curved solenoidal structure, consistent with other LRR proteins that mediate diverse protein-protein interactions[1]. The exact molecular pathways and additional roles outside of flagellated sperm remain under investigation.
