Overview

The liver microenvironment is a complex biological niche composed of diverse cell populations, including hepatocytes, liver sinusoidal endothelial cells (LSECs), Kupffer cells, hepatic stellate cells (HSCs), and various infiltrating immune cells, all embedded within a specialized extracellular matrix (ECM) (PMID: 32665539). It functions as a dynamic system that regulates metabolic homeostasis, blood filtration, and immune tolerance under physiological conditions (PMID: 32555446). In the context of disease, particularly hepatocellular carcinoma (HCC) and cirrhosis, the microenvironment is often 'reprogrammed' into a pro-fibrotic and immunosuppressive state that facilitates tumor growth and evasion of the immune system (PMID: 33431107). Therapeutic strategies frequently aim to modulate specific components of this environment, such as inhibiting angiogenesis via VEGF receptors or reactivating T-cells through checkpoint inhibitors like PD-1/PD-L1 blockers (PMID: 31067457). Because it encompasses a multitude of cell types and signaling pathways rather than a single protein, it is categorized as a biological system or niche rather than a discrete molecular target (PMID: 37033506).
