Overview

Local extracellular pH refers to the acidity or alkalinity of the immediate extracellular environment in tissues. It is not a discrete molecular entity, nor a protein, receptor, or enzyme, but rather a chemical property determined by the balance of acids (mainly protons, or H⁺ ions) and bases in interstitial fluid. The local extracellular pH can fluctuate due to metabolism, oxygen/CO₂ tension, active ion transport, and disease states. Numerous membrane proteins, including proton-sensing G protein-coupled receptors (such as GPR4 and GPR68), ion channels (like acid-sensing ion channels), and transporters (including Na⁺/H⁺ exchanger, monocarboxylate transporter, and bicarbonate transporters), participate in sensing and maintaining local pH homeostasis[3][4][6][8]. Abnormal shifts in extracellular pH are associated with various disease processes, notably acidosis in cancerous or inflamed tissues, and are actively studied as biomarkers and therapeutic modulating factors. However, "local extracellular pH" itself is not a canonical therapeutic target but rather a characteristic of the tissue microenvironment, distinguished from drug-targetable molecules such as receptors or enzymes[8].
