Overview

Luminal iron refers to the pool of unabsorbed dietary or supplemental iron present within the gastrointestinal tract and the immediate mucosal environment (1.1.1, 1.5.4). While iron is an essential micronutrient for host physiological processes such as oxygen transport and DNA synthesis, excess free iron in the lumen is chemically reactive and can catalyze the production of reactive oxygen species (ROS) via the Fenton reaction (1.2.1, 1.5.5). This oxidative stress can lead to lipid peroxidation and ferroptosis of intestinal epithelial cells, compromising the mucosal barrier and exacerbating inflammatory conditions like Ulcerative Colitis and Crohn's disease (1.2.1, 1.2.4). Furthermore, luminal iron serves as a critical growth factor for many enteric pathogens, and its sequestration is a key component of nutritional immunity (1.5.1, 1.5.3). Therapeutic strategies targeting luminal iron involve the use of non-absorbable chelators or sequestering agents to mitigate inflammation and dysbiosis without causing systemic iron depletion (1.5.4). Drugs like deferiprone and experimental agents like DIBI or lactoferrin-based therapies aim to bind this iron, thereby reducing its bioavailability to pathogens and its capacity for oxidative damage (1.3.3, 1.5.1).
