Luminal water refers to the aqueous fluid contained within the internal cavities or lumens of tubular organs, most significantly the gastrointestinal tract and the prostate gland (Schiller et al., 2010). In the intestines, the volume of luminal water is a primary determinant of stool consistency and motility, and it is tightly regulated by the transport of electrolytes across the epithelial barrier (Ford et al., 2014). While luminal water is a physiological substance rather than a specific molecular receptor or enzyme, it serves as the functional endpoint for various pharmacological agents, such as osmotic laxatives (e.g., polyethylene glycol) and secretagogues (e.g., linaclotide), which aim to increase water content to alleviate constipation (Lembo et al., 2011). In the field of diagnostic imaging, the Luminal Water Fraction (LWF) is a specialized MRI-based biomarker used to detect and characterize prostate cancer (Sabouri et al., 2017). Malignant transformation in the prostate typically involves the loss of fluid-filled ductal spaces, resulting in a measurable decrease in LWF, which correlates with tumor aggressiveness (Dregely et al., 2016). Consequently, luminal water is a critical parameter in both the management of functional bowel disorders and the non-invasive assessment of prostatic health.
Modulation of luminal water is achieved through osmotic retention of fluid or by stimulating the active secretion of ions (e.g., chloride and bicarbonate) into the lumen via channels like CFTR or GC-C, which draws water in through osmosis (Schiller et al., 2010; Currie et al., 2014).
