Overview

Lung function" refers to the ability of the lungs to effectively ventilate, facilitate gas exchange, and maintain homeostasis of oxygen and carbon dioxide levels in the blood[4]. It is not a molecular entity but a complex physiological trait determined by multiple anatomical, genetic, and environmental factors, including airway caliber, lung parenchyma function, and respiratory muscle performance[4]. Impairments in lung function are central to the diagnosis, progression, and prognosis of various pulmonary diseases such as COPD, asthma, and interstitial lung diseases[4]. Common measures of lung function used in clinical and research settings include FEV1, FVC, and DLCO. Modulation of lung function is addressed through diverse drug classes acting on molecular targets upstream (e.g., beta-2 adrenergic receptors, muscarinic receptors), but "lung function" itself is not a single druggable receptor, enzyme, transporter, or gene[4].\n\nThere is a fundamental problem with treating "lung function" as a conventional therapeutic target. The proper approach is to specify a relevant molecular or cellular target such as "Beta-2 adrenergic receptor," "Muscarinic acetylcholine receptor M3," or genetic determinants (e.g., "CFTR") underlying pulmonary biology and pathology.
