The lymphatic interstitial space is the fluid-filled region located between tissue cells and the initial lymphatic capillaries, serving as a vital interface for fluid, solute, and macromolecule exchange (StatPearls, 2023). Its primary biological function is the maintenance of tissue fluid homeostasis by facilitating the drainage of interstitial fluid—which contains proteins, metabolic waste, and signaling molecules—into the lymphatic system to be returned to the blood circulation (NIH, 2022). This compartment is also essential for immune surveillance, providing a pathway for immune cells like dendritic cells to migrate to regional lymph nodes (Nature Reviews Immunology, 2018). In clinical contexts, the lymphatic interstitial space is a major route for cancer metastasis and a significant barrier to effective drug delivery, as elevated interstitial fluid pressure in tumors can prevent therapeutic agents from reaching their targets (Nature Reviews Cancer, 2021). While not a molecular target itself, pharmacological agents like hyaluronidase are sometimes used to modify the interstitial matrix to enhance the dispersion and absorption of co-administered drugs (PubChem, 2024). Pathological changes in this space, such as lymphatic obstruction or increased capillary filtration, lead to the accumulation of fluid and the development of lymphedema (StatPearls, 2023).
