The lymphatic vessels and sentinel lymph node reticuloendothelial system (RES) constitute a specialized anatomical and functional network responsible for fluid transport and immune filtration (StatPearls, 2023). The RES within lymph nodes is primarily composed of macrophages and dendritic cells that clear foreign particles and antigens from the lymphatic fluid (PubMed, PMID: 30035257). In clinical oncology, the sentinel lymph node (SLN) is defined as the first node in a lymphatic basin to receive drainage from a primary tumor, serving as a key indicator for metastatic spread (National Cancer Institute, 2023). Diagnostic agents like Technetium Tc 99m tilmanocept are engineered to target specific molecular components within this system, such as the mannose receptor (CD206) expressed on RES macrophages (FDA, 2013). Other agents, like sulfur colloids, rely on the non-specific phagocytic activity of the RES for localization (PubMed, PMID: 11030592). Mapping this system is essential for the staging of cancers such as melanoma and breast cancer, though it presents challenges related to anatomical variability and the risk of lymphedema following node removal (PubMed, PMID: 25635393).
