Overview

Lymphocyte percentage is a clinical laboratory parameter that represents the proportion of lymphocytes—including T cells, B cells, and natural killer cells—relative to the total white blood cell count in the peripheral blood (StatPearls, 2023). It is routinely measured as part of a complete blood count (CBC) with differential to evaluate the functional status of the immune system (Mayo Clinic, 2023). Although it is not a specific protein, receptor, or therapeutic target, it serves as a vital biomarker for diagnosing and monitoring various physiological and pathological states (NCI Dictionary, 2024). Elevations in lymphocyte percentage, known as lymphocytosis, often occur in response to viral infections or chronic lymphocytic leukemia, while a decrease, known as lymphopenia, can result from malnutrition, severe stress, or the use of immunosuppressive therapies. Many pharmacological agents, such as corticosteroids and monoclonal antibodies targeting B-cell antigens, exert their therapeutic effects by altering lymphocyte populations, which is subsequently reflected in this clinical measurement. Therefore, monitoring the lymphocyte percentage is essential for assessing both disease progression and the safety profiles of immunomodulatory drugs.
