Overview

M protein is the major surface-associated virulence factor of Group A Streptococcus (Streptococcus pyogenes) and plays a central role in the pathogenicity of the bacterium[1][2][5]. It is a fibrillar, coiled-coil dimeric protein anchored in the bacterial cell wall, with its highly variable N-terminus extending into the extracellular environment and interacting with numerous host proteins such as fibrinogen, C4b-binding protein, and fibronectin[3][1][4]. M protein promotes immune evasion by inhibiting phagocytosis and interfering with complement activation, and it aids in adhesion to epithelial cells[1][2][5]. It is also a key determinant in serotyping and epidemiological tracking of GAS strains via emm typing[5]. Immune responses against M protein are complex—while antibody generation can enhance opsonization and bacterial clearance, cross-reactive antibodies can contribute to autoimmune sequelae such as acute rheumatic fever[2][1]. M protein is a major focus of GAS vaccine development due to its dominance as a target of the host immune system and its critical role in virulence[2][3][5].
