Major histocompatibility complex (MHC) class I and II molecules presenting Epstein-Barr virus (EBV) peptides are essential components of the immune system's ability to detect and eliminate EBV-infected cells (PubMed: 21454471). These complexes form when viral proteins, such as EBNA or LMP, are processed and loaded onto HLA molecules for surface display. CD8+ cytotoxic T cells recognize MHC class I-peptide complexes, while CD4+ helper T cells recognize MHC class II-peptide complexes (StatPearls: EBV). In EBV-associated malignancies like nasopharyngeal carcinoma and post-transplant lymphoproliferative disorder (PTLD), these complexes serve as specific markers for therapeutic intervention. Adoptive T-cell therapies, such as tabelecleucel (Ebvallo), are designed to specifically bind these MHC-peptide targets to induce cell lysis (EMA: Ebvallo). Beyond oncology, these complexes are being investigated in autoimmune contexts, particularly multiple sclerosis, where EBV-infected B cells are thought to play a pathogenic role (Atara Bio: ATA188). Therapeutic challenges include the high diversity of HLA alleles across populations and the ability of EBV to downregulate MHC expression to evade detection. Monitoring EBV viral load and HLA compatibility is crucial for the success of treatments targeting these molecules.
