NY-ESO-1 (New York esophageal squamous cell carcinoma 1), encoded by the CTAG1B gene, is a prominent cancer-testis antigen (CTA) that is highly immunogenic (UniProt: P12101). In healthy adults, its expression is strictly limited to the germ cells of the testis and placenta, which lack MHC class I expression, effectively making the protein invisible to the immune system under normal conditions (PMID: 9045611). However, NY-ESO-1 is aberrantly expressed in numerous cancers, including synovial sarcoma and melanoma, where its intracellular proteins are processed into peptides and presented on the cell surface by MHC class I and II molecules (PMID: 16322273). The most common targetable epitope is the HLA-A*02-restricted SLLMWITQC peptide, which is the focus of several TCR-engineered T-cell therapies like Afamitresgene autoleucel (PMID: 11145915). These therapeutic interventions leverage the high specificity of the TCR for the peptide-MHC complex to selectively eliminate malignant cells while sparing healthy somatic tissues. Monitoring for HLA type and NY-ESO-1 expression levels is critical for patient selection in these precision immunotherapy approaches (FDA, 2024).
T-cell receptor (TCR) binding to the specific NY-ESO-1 peptide-MHC complex triggers T-cell activation, cytokine release, and directed granzyme/perforin-mediated cytotoxicity against the antigen-presenting tumor cells.
