The Major surface glycoprotein (MSG), also known as gp120, is the primary surface component of the opportunistic fungus Pneumocystis jirovecii. It plays a critical role in the pathogenesis of Pneumocystis pneumonia (PCP) by mediating the attachment of the fungus to alveolar type I pneumocytes via interactions with host proteins like fibronectin and surfactant protein D (StatPearls, 2023). MSG is characterized by extreme genetic diversity; the organism contains over 100 MSG genes, but only one is expressed at a time at a specific genomic site called the expression locus (UniProt). This mechanism allows for rapid antigenic variation, enabling the pathogen to evade the host's immune response and persist in the lungs (PubMed, 2021). While current standard-of-care treatments for PCP, such as trimethoprim-sulfamethoxazole, target metabolic enzymes like dihydrofolate reductase, MSG is a primary target for diagnostic antibody-based assays and is a major focus for vaccine development and monoclonal antibody therapies (NIH, 2022).
