Megakaryoblasts exposed to carboplatin represent a specific cellular state or experimental model rather than a single molecular target. Megakaryoblasts are the earliest identifiable precursors in the megakaryocytic lineage, which undergo endomitosis and maturation to produce platelets essential for blood clotting (StatPearls, 2023). Carboplatin is a platinum-based chemotherapeutic agent that exerts its cytotoxic effects by forming covalent DNA adducts and cross-links, which inhibit DNA replication and transcription (PubChem, CID 426756). When these progenitor cells are exposed to carboplatin, the resulting genomic instability often leads to cell cycle arrest or apoptosis, a process that is a primary driver of chemotherapy-induced thrombocytopenia (CIT) (PubMed, PMID 28451545). This condition is clinically significant as it often necessitates chemotherapy dose reductions or delays, impacting cancer treatment efficacy. Research into this state focuses on identifying protective agents or thrombopoietin (TPO) receptor agonists, such as romiplostim or eltrombopag, to mitigate the bone marrow toxicity associated with platinum-based regimens (NIH, 2022).
Carboplatin induces DNA intra-strand and inter-strand cross-links in megakaryoblasts, which triggers DNA damage checkpoints and p53-mediated apoptosis, ultimately leading to a reduction in mature megakaryocytes and systemic platelet production.
