Overview

Menthol is a cyclic monoterpenoid alcohol found primarily in peppermint and other mint oils, existing mainly as the (–)-menthol stereoisomer in nature. It is widely used in the pharmaceutical industry as a topical astringent, local anesthetic, and counterirritant, most notably in products designed to provide a cooling sensation and relieve minor throat or skin irritation. At the molecular level, menthol acts primarily as an agonist at the TRPM8 ion channel, a cold-sensitive receptor found in sensory neurons, explaining its cooling effects. It also has some activity at the κ-opioid receptor and has demonstrated modest preclinical antifungal and antibacterial activity. Menthol cannot be considered a biological drug target; rather, it should be classified as an active small molecule ligand.

Mechanism of action

Agonist of the TRPM8 ion channel (the "cold and menthol receptor") in sensory neurons, leading to a cooling or numbing sensation. Weak agonist at the κ-opioid receptor. Membrane effects altering cell surface tension and permeability (antifungal, antibacterial activities, preclinical). Topical counterirritant via peripheral sensory modulation.
