Overview

Mesenchymal stem cells (MSCs) possess significant immunomodulatory and anti-inflammatory properties. These effects are mediated through both direct cell-cell interactions and the secretion of soluble factors, modulating both innate and adaptive immune responses. Key secreted molecules include IDO, PGE2, IL-10, TGF-β, and TSG6. MSCs act as "guardians" at sites of injury or inflammation, controlling local immune responses and facilitating tissue repair. They are being explored for treating inflammatory diseases, but their therapeutic efficacy can be inconsistent.

Mechanism of action

MSCs exert their anti-inflammatory effects through the secretion of soluble factors (IDO, PGE2, IL-10, TGF-β, TSG6) and direct cell-cell interactions, modulating both innate and adaptive immune responses. This leads to suppression of effector T cell activation/proliferation, promotion of regulatory T cells, reduction in B-cell proliferation and antibody production, polarization of macrophages toward an M2 phenotype, and inhibition of dendritic cell and NK cell activity.
