Overview

The mesolimbic dopamine pathway, commonly known as the brain's reward pathway, is a vital neural circuit that regulates motivation, reinforcement learning, and the experience of pleasure [5, 11]. It primarily originates from dopaminergic neurons in the ventral tegmental area (VTA) and extends to the nucleus accumbens (NAc), which serves as a central hub for reward processing [8, 18]. Under physiological conditions, the release of dopamine within this pathway reinforces life-sustaining behaviors such as eating, social interaction, and reproduction [1, 11]. However, this system is significantly hijacked by drugs of abuse, which can trigger abnormally large increases in synaptic dopamine, leading to the neuroadaptations that characterize addiction [12, 14]. Dysregulation of this pathway is also fundamentally linked to various psychiatric and neurological disorders; for instance, dopaminergic overactivity is a hallmark of schizophrenia, while deficits are associated with the anhedonia seen in major depressive disorder [4, 7]. Pharmacological agents typically modulate this pathway by targeting its constituent proteins, such as dopamine receptors or the dopamine transporter (DAT), to restore normal signaling [2, 9].

Mechanism of action

Drugs modulate the pathway by increasing synaptic dopamine concentrations through the inhibition of the dopamine transporter (DAT), promotion of presynaptic dopamine release, or by acting as direct agonists or antagonists at dopamine receptors (D1-D5) within the circuit [3, 9, 14].
