Overview

**MicroRNA 4523** (miR-4523, MIR4523) is a small, non-coding RNA molecule in humans that functions primarily as a post-transcriptional regulator of gene expression. It binds to target mRNAs, most notably phosphoglycerate kinase 1 (PGK1), causing their silencing and degradation. This regulatory action activates the Nrf2 signaling cascade, ultimately offering cytoprotection to osteoblasts against dexamethasone-induced oxidative stress and apoptosis. Reduced levels of miR-4523 have been associated with increased vulnerability to osteonecrosis, particularly in the context of prolonged corticosteroid treatment, making it a potential biomarker for disease risk or progression. There are currently no drugs that directly target miR-4523, and it is not generally considered a classic \"therapeutic target\" akin to receptors or enzymes, but rather a regulatory molecule with potential utility in biomarker discovery and experimental therapeutics[2].
