microRNA 4759 (miR-4759) is a small, non-coding RNA belonging to the microRNA family. Identified as a novel direct negative regulator of PD-L1 in breast cancer, miR-4759 binds to two conserved sites within the 3′-UTR of the PD-L1 gene and inhibits its expression. Overexpression of miR-4759 in breast cancer cells sensitizes them to cytotoxic T-cell–mediated killing, suppresses tumor growth in immunocompetent mice, and is associated with better overall survival in human breast cancer. miR-4759 is predominantly downregulated in breast tumors versus normal tissue, particularly in triple-negative subtypes. Its primary therapeutic interest is as an immune-sensitizing agent, with clinical application being the delivery of miR-4759 mimics to suppress PD-L1 and enhance anti-tumor immunity
