Overview

The microvascular and capillary endothelium is a specialized monolayer of cells that lines the smallest blood vessels, including capillaries, arterioles, and venules, serving as a critical interface between the blood and interstitial fluid (Aird, 2007). It plays a fundamental role in physiological processes such as the regulation of vascular tone, nutrient and gas exchange, and the recruitment of leukocytes during inflammatory responses (Pober & Sessa, 2007). In pathological conditions, particularly oncology, the microvascular endothelium is a primary driver of tumor growth and metastasis through the process of angiogenesis, where new vessels are formed to sustain the metabolic demands of the tumor (StatPearls, 2023). Therapeutic strategies targeting this compartment include anti-angiogenic agents that inhibit growth factor signaling (e.g., VEGF inhibitors) and vascular disrupting agents that cause the rapid collapse of established tumor vessels (NIH, 2022). Beyond cancer, microvascular dysfunction is central to the pathogenesis of diabetic microangiopathy, hypertension, and acute respiratory distress syndrome (ARDS), where loss of barrier integrity leads to edema and organ failure. Consequently, monitoring endothelial health through biomarkers like von Willebrand factor or circulating endothelial cells is vital for assessing disease progression and treatment efficacy.
