Monoamine oxidase B (MAO-B) is a mitochondrial enzyme primarily known for its role in the breakdown of neurotransmitters like dopamine. In the context of neurodegenerative conditions such as Alzheimer's disease, MAO-B becomes significantly upregulated in reactive astrocytes. This 'novel' pathological role involves the aberrant production of GABA and hydrogen peroxide, which leads to tonic inhibition of neighboring neurons and oxidative stress. Modern therapeutic strategies, such as the development of the reversible inhibitor KDS2010, aim to selectively block astrocytic MAO-B to reverse cognitive decline and slow disease progression. Unlike traditional MAO inhibitors used in Parkinson's, these next-generation candidates focus on the specific metabolic shift in glia rather than just neurotransmitter preservation.
