Overview

Mucosal epithelial tight junction complexes, also known as zonula occludens, are specialized intercellular structures that form a continuous seal between adjacent epithelial cells [StatPearls, 2023]. They serve as a critical semi-permeable barrier that regulates the paracellular movement of ions, water, and macromolecules while maintaining cellular polarity [NCBI, 2020]. These complexes are composed of transmembrane proteins such as claudins, occludin, and junctional adhesion molecules (JAMs), which are anchored to the actin cytoskeleton via scaffold proteins like zonula occludens-1 (ZO-1) [UniProt, 2024]. Dysregulation of tight junctions is a hallmark of various "leaky barrier" diseases, including celiac disease and inflammatory bowel disease, where increased permeability allows the entry of luminal antigens and pathogens [PubMed, 2021]. Therapeutically, these complexes are targeted either to restore barrier integrity, as seen with zonulin antagonists like larazotide acetate, or to temporarily increase permeability for enhanced drug delivery [ClinicalTrials.gov, 2022]. Understanding the molecular architecture of these junctions is essential for developing treatments that can precisely modulate mucosal permeability without compromising systemic safety [Nature Reviews Drug Discovery, 2019].
