Mucosal immune balance refers to the complex physiological state of homeostasis maintained at mucosal surfaces, such as the gastrointestinal tract, respiratory tract, and urogenital tract. This balance is characterized by a controlled equilibrium between protective immune responses against pathogens and active tolerance toward commensal microbiota and harmless environmental antigens (Belkaid & Hand, 2014, Science). It involves a highly coordinated network of epithelial cells, innate immune cells like dendritic cells, and adaptive immune cells, particularly the balance between pro-inflammatory Th17 cells and anti-inflammatory regulatory T cells (Tregs) (Mowat & Agace, 2014, Nature Reviews Immunology). Dysregulation of this balance, often resulting from genetic susceptibility, environmental triggers, or dysbiosis, is a central driver in the pathogenesis of chronic inflammatory conditions such as inflammatory bowel disease (IBD) and various allergies (Neurath, 2014, Nature Reviews Immunology). While 'Mucosal immune balance' is not a single molecular target, it is the primary therapeutic goal for many drugs. Modern biologics and small molecules target specific components of this system—such as TNF-alpha, Janus kinases, or integrins—to suppress pathological inflammation and restore the homeostatic state (Danese et al., 2015, Gut). Achieving and maintaining this balance is critical for preventing tissue damage while preserving the host's ability to defend against infections.
Therapeutic intervention aims to restore mucosal immune balance by inhibiting pro-inflammatory cytokines (e.g., TNF-alpha, IL-12/23), blocking leukocyte trafficking to mucosal tissues (e.g., alpha-4 beta-7 integrin inhibition), or modulating the gut microbiota to favor anti-inflammatory regulatory T-cell (Treg) activity.
