Multi-characteristic opsin 020 (MCO-020) is a synthetic, chimeric opsin protein designed for optogenetic vision restoration in patients with advanced retinal degeneration, specifically geographic atrophy (GA) secondary to age-related macular degeneration (Nanoscope Therapeutics, 2024). Unlike traditional optogenetic tools that require high-intensity light or specific wavelengths, MCO-020 is engineered to be sensitive to broad-spectrum ambient light, making it more practical for daily use (Nanoscope Therapeutics, 2024). The protein is delivered to the retina via an adeno-associated virus (AAV) vector and is typically targeted for expression in surviving inner retinal neurons like bipolar cells. Upon binding with the endogenous chromophore all-trans retinal, MCO-020 forms a functional light-activated complex that initiates neural signaling in response to light, effectively substituting for the function of lost rod and cone photoreceptors (ClinicalTrials.gov, 2023). This approach aims to restore functional vision and improve quality of life for patients who have lost significant sight due to the progression of GA (Nanoscope Therapeutics, 2024).
MCO-020 is an optogenetic therapy that delivers a gene encoding a light-sensitive chimeric opsin to retinal cells. Once expressed, the opsin binds to endogenous all-trans retinal to form a functional photopigment. This complex enables surviving retinal neurons (such as bipolar cells) to respond to ambient light, triggering depolarization and subsequent neural signaling to the brain, thereby bypassing lost photoreceptors (Nanoscope Therapeutics, 2024; ClinicalTrials.gov, 2023).
