The multifidus muscle is a deep spinal muscle comprising part of the transversospinal group, present from the cervical to the lumbar spine. Its fascicles span several vertebral levels, originating from vertebral processes and attaching to spinous processes above. It stabilizes the spine during movement by controlling extension, flexion, and rotation, and is critical for segmental spinal stability. Dysfunction, atrophy, or impaired activation are closely linked to chronic low back pain and spine instability, often visualized as muscle wasting on imaging. Rehabilitation is challenging due to difficulty isolating and training the multifidus effectively. "Neuromuscular activation" of multifidus refers to the pattern and integrity of muscle activation via its innervation and neural pathways, a concept relevant for spine health but not a defined therapeutic molecule or receptor target.
