Multiple endogenous pathways via paracrine signaling and immunomodulation is a descriptive term for a complex biological mechanism rather than a specific molecular target. This process is primarily associated with the therapeutic action of Mesenchymal Stem Cells (MSCs) and other cell-based therapies, which exert their effects by secreting a variety of bioactive molecules—including cytokines, chemokines, and growth factors—that influence neighboring cells (paracrine signaling) and modulate the immune system (Galipeau & Sensébé, 2018, Nature Medicine). These secreted factors, often referred to as the secretome, work through multiple signaling cascades to promote tissue repair, inhibit apoptosis, and reduce inflammation (Caplan, 2017, Stem Cells Translational Medicine). Because it involves a broad array of interactions across different pathways, it cannot be localized to a single receptor or enzyme. This mechanism is critical in treating multi-factorial diseases such as graft-versus-host disease, myocardial infarction, and various autoimmune conditions (Pittenger et al., 2019, NPJ Regenerative Medicine). Consequently, while it is a target of therapeutic intervention, it does not constitute a single target in the traditional pharmacological sense. Therapeutic strategies leveraging these pathways often focus on the bystander effect where the primary cells do not necessarily integrate into the tissue but rather orchestrate a healing response through their secretome.
The mechanism involves the coordinated secretion of diverse bioactive factors that act on local cell populations to alter the microenvironment, suppress inflammatory cell activity, and activate endogenous regenerative programs.
