Overview

The term 'multiple intracellular mRNAs and signaling pathway components' refers to the broad, pleiotropic molecular interactome of defibrotide, rather than a single, specific therapeutic target (DrugBank, DB09207). Defibrotide is a complex mixture of porcine-derived polydeoxyribonucleotides that exerts its effects by binding to and modulating the expression of various intracellular messenger RNAs and signaling proteins within endothelial cells (FDA Label, 2016). These interactions result in the downregulation of pro-inflammatory and pro-coagulant factors while upregulating fibrinolytic components, such as prostaglandin I2 and E2 (EMA, 2013). This multi-target mechanism is clinically utilized to treat hepatic veno-occlusive disease (VOD), also known as sinusoidal obstruction syndrome (SOS), which is a life-threatening complication of hematopoietic stem cell transplantation (StatPearls, 2023). Because this 'target' represents a collection of diverse molecular entities rather than a discrete protein or receptor, it is considered a descriptive pharmacological profile used to explain the drug's complex polypharmacology (PubMed: 27533344).

Mechanism of action

Defibrotide interacts with multiple intracellular mRNAs and signaling pathway components to modulate endothelial cell function, increasing fibrinolysis and reducing pro-inflammatory and pro-coagulant states (FDA Label, 2016).
