Multiple lymphocyte and immune cell surface antigens refers to a heterogeneous group of proteins expressed on the surface of various hematopoietic and endothelial cells. This collective target profile includes CD2, CD3, CD4, CD8, CD11a, CD18, CD25, CD44, CD45, and HLA Class I and II molecules (Sanofi, 2022, FDA Label: Thymoglobulin). These molecules are involved in essential biological processes such as T-cell activation, leukocyte adhesion, and immune signaling (Mohty, 2007, Leukemia). In therapeutic applications, these antigens are collectively targeted by polyclonal antibody preparations like Antithymocyte Globulin (ATG) (Gabardi et al., 2006, AJHP). The primary mechanism of action involves the depletion of circulating T-lymphocytes through complement-dependent cytotoxicity and apoptosis (Popow et al., 2013, American Journal of Transplantation). This broad targeting is clinically utilized to prevent and treat acute organ transplant rejection and graft-versus-host disease (GVHD) (Zand et al., 2005, Transplantation). Because the target set is broad, therapy often results in profound lymphopenia and requires careful monitoring of immune cell counts (NIH, 2023, LiverTox). Safety concerns associated with hitting these multiple targets include cytokine release syndrome, serum sickness, and an increased risk of opportunistic infections (StatPearls, 2023, Antithymocyte Globulin).
Polyclonal antibodies bind to a wide array of surface antigens, leading to T-cell depletion via complement-dependent cytotoxicity (CDC), antibody-dependent cellular cytotoxicity (ADCC), and apoptosis, while also modulating adhesion molecules and signaling receptors.
