Overview

The processes involved include host-side enzymes such as Inosine Monophosphate Dehydrogenase (IMPDH), which regulates the guanosine triphosphate (GTP) pool, and viral-side enzymes like RNA-dependent RNA polymerase (RdRp) (Thomas et al., 2011). By disrupting these multiple processes, therapeutic agents can inhibit viral RNA capping, suppress genome replication, and induce lethal mutagenesis, where the viral mutation rate is increased beyond a sustainable threshold (Crotty et al., 2000). While this multi-target approach is effective against a wide range of RNA viruses and reduces the likelihood of resistance, it often results in a narrow therapeutic index due to the essential nature of nucleotide metabolism in healthy host cells. Consequently, drugs targeting these processes are frequently associated with significant systemic toxicities, most notably hemolytic anemia (FDA Label, Rebetol).
