Overview

Multivalent metal cations are positively charged ions with a valence of +2 or higher, including essential physiological elements like calcium (Ca2+), magnesium (Mg2+), iron (Fe2+/3+), and zinc (Zn2+), as well as toxic heavy metals like lead (Pb2+) and mercury (Hg2+). These ions are fundamental to human biology, acting as structural components of the skeletal system, essential cofactors for thousands of enzymatic reactions, and critical messengers in intracellular signaling pathways (NIH, 2022). In clinical pharmacology, they are highly significant due to their ability to form stable, often insoluble complexes with various drugs, such as tetracycline antibiotics and fluoroquinolones, which can drastically impair drug absorption and therapeutic efficacy (StatPearls, 2023). Therapeutically, these cations are targeted by chelating agents like deferoxamine or EDTA to treat metal poisoning or iron overload by sequestering the ions for renal or biliary excretion (Mayo Clinic, 2023). Additionally, multivalent cations are utilized as active ingredients in antacids, laxatives, and mineral supplements to manage deficiencies or conditions like hyperphosphatemia in chronic kidney disease (PubChem, 2024).
