Overview

Instead of targeting a single protein like BCMA or CD38, this approach utilizes the full spectrum of antigens expressed by an individual patient's plasma cells, including cancer-testis antigens (e.g., MAGE, NY-ESO-1) and patient-specific neoantigens. By delivering the genetic blueprint of these antigens via RNA, the immune system is trained to recognize and eliminate malignant cells through a multi-antigenic T-cell response. This strategy aims to overcome tumor heterogeneity and prevent immune escape, which are common challenges in the treatment of multiple myeloma. Clinical applications often involve harvesting a patient's own tumor RNA and loading it into autologous dendritic cells to trigger a robust, durable anti-tumor immunity (PMID: 15681738, PMID: 21454461).

Mechanism of action

The mechanism involves the delivery of tumor-derived RNA (either total RNA or specific mRNA sequences) into antigen-presenting cells, typically dendritic cells. These cells then translate the RNA into proteins, process them into peptides, and present them on MHC molecules to stimulate a polyclonal T-cell response against multiple myeloma-associated antigens simultaneously (PMID: 11830466, PMID: 25533491).
