Overview

Myocardial fatty acid uptake imaging agents are radiolabeled molecules used to visualize and quantify the uptake of fatty acids by cardiac muscle. These agents are used in nuclear medicine, primarily with PET or SPECT imaging, to assess myocardial metabolism, viability, and detect metabolic dysfunction in various heart diseases. They mimic natural long-chain fatty acids (LCFAs) and are transported into cardiomyocytes via fatty acid transporters such as FAT/CD36. Once inside cells, they may undergo partial steps of β‐oxidation or be metabolically trapped, allowing visualization proportional to cellular FA uptake/metabolism. Altered tracer distribution can indicate regions with impaired substrate utilization before anatomical changes become apparent.
