Overview

N-acetylneuraminic acid (Neu5Ac) is the predominant sialic acid found in human cells, typically positioned at the terminal ends of glycan chains on surface glycoproteins and glycolipids. It plays a vital role in biological recognition and immune homeostasis, acting as a 'self' marker that interacts with Siglec receptors to prevent autoimmune activation (PubChem CID 439197). Neu5Ac is a well-characterized target in infectious disease because it serves as the essential attachment receptor for the influenza virus hemagglutinin protein (PubMed: 22230605). Furthermore, in oncology, many tumors exhibit hypersialylation, a state of elevated Neu5Ac density that masks cancer cells from immune detection and promotes metastasis (PubMed: 30141381). From a pharmacological perspective, Neu5Ac is the chemical template for neuraminidase inhibitors, which block the enzymatic release of viral particles, and it is the target of therapeutic sialidases like DAS181 that strip the sugar from host cells to prevent infection. Current biotech research continues to target Neu5Ac pathways to develop novel anti-adhesion therapies and cancer immunotherapies.

Mechanism of action

Competitive inhibition of viral neuraminidase enzymes by structural mimicry of the N-acetylneuraminic acid transition state; enzymatic cleavage of terminal N-acetylneuraminic acid residues from respiratory epithelium to prevent viral binding; modulation of sialic acid-Siglec interactions to enhance immune response.
